Confidentiality/ Privacy:
Medical information is confidential and will not be released without the patient's written consent except as required by law.

Under HIPPA and FERPA regulations, health care providers cannot speak to parents, spouses, insurance agencies, or other parties without direct permission from patients over 18 years of age. Patients under 18 cannot be treated through KSU Health Services without a signed Parental Permission Form.

KSU Student Health Services Policy

 

Fees:
Student Health Fees, $51 for Spring or Fall semesters and $26 per Summer semester, pays for free clinic visits without a copay charge. Any fees incurred from tests, injections, or medication will be billed to the patient’s Owl Express account and must be paid through the Bursar's office within 30 days.

We do not file insurance claims or provide medical codes to insurance companies. We are considered an "out of network" provider; costs incurred with KSU Student Health Services typically are not reimbursed.

 

Medical Release Form:
If patients are transferring care from another provider or need records sent to KSU Health Services, they may be asked to fill out the Medical Release Form. Patients should have the name and contact information of their previous providers available.

Patients may request a copy of their records several ways:

For MEDICAL Records ONLY:  Download and complete the Medical Release Form; make sure to SIGN the form (no electronic signatures will be accepted). Make a copy of a government-issued photo ID or current student ID card and fax it with the signed form to (770) 499-3655.

KSU Student Health Services will provide one copy of a student's entire file at no charge. Any additional requests are subject to a $20 search and retrieval fee and a $.65 per page copy charge.
By law (O.C.G.A. 50-18-70 through 50-18-72), KSU Student Health Services has three days to respond to any records request. No records are released the same day.
