Deadlines:
ALL required forms, listed above, are required by the listed due date for the semester in which you are doing your Clinical.
Fall 2008 - July 11, 2008
Spring 2009 - Nov. 14, 2008

If any of your credentials (ie - PPD, CPR, Physical, blood borne) are due to expire during a semester (regardless if done within the year), you are required to update it before the above listed deadlines for that semester. It is the student's responsibility to find out what Medical information is pending.
