DO NOT SUBMIT ANY PDF FORMS OR DOCUMENTS TO CHOA

  1. Read EXHIBIT A, Children’s Participation Agreement (DO NOT SUBMIT) link
  2. Review EXHIBIT C, Participant Health Screen Requirements (DO NOT SUBMIT) link
  3. IF Health Screen Requirements have been met, please email nu_medrecords@kennesaw.edu to sign EXHIBIT B-1, Participant Agreement Tracking Form
  4. Complete Pre-Clinical Orientation Checklist (DO NOT SUBMIT TO CHOA) link
    • Student Name: Print your first and last name
    • Student ID# or last 4 digits of SSN#: Print your KSU Student ID #
    • School Name: Kennesaw State University
    • Instructor: Leave Blank
    • Unit & Location: Print your assigned location: Egleston, Hughes Spaulding or Scottish Rite
    • Qtr/Semester: Print the semester (Spring, Summer or Fall) and the year
    • Date: Today’s Date
    • Completion Date: Fill in the date you completed each module
  5. Submit Pre-Clinical Orientation Checklist (Item 4) and a copy of flu shot for current flu season to Room 3004.
  6. Log into Advantage Students account and select Children’s Health Care of Atlanta to view your results.

 

Parking Requirements:

  • Read over the parking information for Clinical Groups ( class 3313 or 3318)  Practicum/Precetorship Students (4417)
  • Your clinical instructor will give you a CHOA badge that will grant you access to the employee parking deck Entrance/Exit.
