  1. Read the WellStar Health System Orientation Packet (GAMES 2012)
  2. Confidentiality & Technology Acceptance Agreement Form
    • Signature: Sign your name
    • Print Name: Print your name
    • Date: Today’s date
    • Employee#/Student/ Faculty ID#: Your KSU ID#
  3. Complete test and correct with Exam key - Repeat test until scoring a grade of 25/28 or greater
  4. Faculty and Student Orientation Confirmation
    • Name: Print your name
    • School: Kennesaw State University
    • Facility: Kennestone, Cobb, Windy Hill, Douglas or Paulding
    • Department, Beginning and Ending Date. Nursing Staff will complete.
    • Student/Faculty Signature: Sign your name
  5. Log into your Advantage account and select WellStar Hospital to view your results   
  6. Dress code: Tennis shoes with mesh are no longer permitted, they must be all leather
  7. Cobb Hospital Parking
