These are the immunizations that you need to prove to be a student at KSU. You may use either our form signed by a doctor or nurse practitioner or an official document from a clinic, county health department, or other source. This document must be submitted in person, mailed, or faxed to the KSU Immunization Department located in the KSU Health Clinic.


  1. 2 MMR's (measles, mumps or rubella)
  2. A tetanus shot within the past 10 years
  3. 2 doses of the chicken pox (varicella) vaccine spaced at least 3 months apart if both doses are given before your 13th Birthday, or 2 doses at least 4 weeks apart if the first dose is given after your 13th birthday. If you have already had chicken pox, just let us know the year you had it. You can simply write it in the "Date of Disease" box under "Varicella". (This is the one that most people forget to include with their immunization information.)
  4. 3 doses of the hepatitis B vaccine if you are 18 or under when you enter KSU

If you were born before 1957 you are considered immune to measles, mumps and rubella. You can just insert your birthday in the spaces on our form for MMR #1 and #2. You still have to supply information about varicella and tetanus.


 

If you will be living in the residence halls on campus you must supply proof of meningitis vaccine to the housing office. Information can be found at www.kennesaw.edu/residencelife/.

Frequently Asked Questions (FAQ's)

Q: I just tried to register and was told I have an immunization hold. What does this mean?

A: This means that you need to show proof of an immunization. You should have received an email from the immunization coordinator that lists any holds that you have. The immunization holds that are listed can be MMR, Tetanus, Varicella (Chickenpox) or Hepatitis B. You will need to bring proof of immunization to the immunization coordinator located in House 52, Student Health Clinic, and your hold will be removed.

Q: Where do I find proof?     

A: Your immunization information may be found in:

  • County Health Dept. or your Yellow Immunization Card
  • High school transcripts (you will need to check with the high school you attended to see if they still have the records)
  • Military medical records
  • Medical records which show receipt of the vaccine with the dates, the provider/nurse signature and the clinic stamp
  • Laboratory evidence of immunity. When showing proof for MMR, this means that you must have a lab report which shows immunity for Measles, Mumps and Rubella. For Hepatitis B, you must have the laboratory report which shows immunity to Hepatitis B.

Q: I can't find my proof and I don't want to be immunized. What can I do? 

A: You can obtain laboratory proof of immunity through a blood test. You can request this from your private medical provider or make an appointment for the test at the Health Clinic. You will need a laboratory report for all three, measles, mumps and rubella; or for Hepatitis B if you are 18 years of age or younger.

Q: I am 18 years old, am I still required to show proof or get the Hepatitis B vaccine?

A: Yes. If you are 18 years of age or younger at the start of your entering academic year at Kennesaw State University, you must show proof that you have had all three injections. If you have not started the vaccination or in the process of completing, you may make an appointment to start or continue the vaccine.

Q: I am not finished with the Hepatitis B vaccine, can I still show proof?     

A: Yes. If you are in the middle of completing of your vaccine, you must bring us proof that you've had at least one or two Hepatitis B vaccinations so that we may clear the registration hold temporarily until you have completed the series. We cannot temporarily waiver the hold until you have an actual registration hold on your record.

Q: I am a transfer student from another USG school and have showed proof of immunization requirements already at these colleges, will my records automatically transfer over?

A: If you are transferring from another University, your immunization records will not be automatically transferred over to Kennesaw State University. Therefore, if you do not show proof of the required immunizations, your registration will be held for the following semester. You may call the student health center at your previous University to obtain those records.

Q: I am a transfer student, am I required to have the Hepatitis B vaccine?

A: If you are 18 years or younger at the time of your transfer to Kennesaw State University, you will need to provide proof of the Hepatitis B vaccine series. If you are 19 years or older at the time of transfer to Kennesaw State University, it is not required that you must have the Hepatitis B vaccination. It is only required that you must show proof of your MMR vaccines, a tetanus vaccine in the last 10 years and either the year you had varicella (chicken pox) or proof that you have been immunized against the disease.

Q: I am transferring to another school and I need my immunization records, what do I do?

A: Visit the Patient Information page, download the Medical Records Release Form, fill it out and fax it back to 678-797-2224. By law, KSU has 3 days to turn around records requests. They are usually done in 24 hours. You can also contact the immunization coordinator and pick up your records in person.
