1. How do I declare a major in the Department of Health, Physical Education, and Sport Science?
To declare a major in the HPS Department at KSU, all students must complete the Program of Study Change Form (available in CC 3015) and present it to Mrs. Lynn Samples (CC 3015 or 770-423-6216).
2. How do I find out who is my academic advisor?
All new students seeking academic advisement should contact Mrs. Amy Howton (CC 3008 or 770-420-4402) when they officially declare their major of study with Mrs. Lynn Samples (CC 3015). Mrs. Amy Howton will assign each new major a faculty advisor.
3. Whom should I contact to request to be added to a closed class outside of the Department of HPS (e.g., ENGL 1101, MATH 1101, etc)?
Contact the appropriate Department Chair for the specific course you are trying to add.
4. Whom should I contact to request to be added to a closed class within the Department of HPS (e.g., HPS 1000, HPS 3200, etc)?
Contact the HPS Department's Central Office (CC 3003 or CC 3015) for the departmental form. Have your advisor sign the form and then return it to the HPS Department's Central Office (CC 3003 or CC 3015). Forms will not be accepted via FAX or electronically.
5. Where can I find more information on the Exercise and Health Science major?
Contact Mrs. Amy Howton (CC 3008 or 770-420-4402) or go to the Exercise and Health Science Major web site.
6. Where can I find more information on the Health and Physical Education (P-12) Teacher Education major?

7. Where can I find more information on the Sport Management major?

8. I am returning to school, whom do I need to see?
If you are a returning student or have been formally admitted to KSU, contact Mrs. Amy Howton (CC 3008 or 770-420-4402) for the specific degree program in which you are interested.
If you are new to KSU you should contact the Office of Undergraduate Admissions to begin your application process.
9. Where do I pick-up forms for Program Admission, Student Teaching, Practicum, Internship, etc.?
For the majority of HPS forms contact Mrs. Lynn Samples, Administrative Secretary (CC 3015). For the Application for Student Teaching, contact Ms. Janice Traylor, Center for Field Experiences and Partnerships (KH 3209). For the application for GACE exams, go to the GACE web site.
** Beginning in Fall 2006, the PRAXIS I and II exams
will be replaced by the Georgia Assessments
for the Certification of Educators (GACE).

10. Does KSU offer a Physical Therapy Program?
KSU does not offer a degree program in Physical Therapy. However, students often attend KSU majoring in either Exercise and Health Science or Biology. Following completion of this degree, students have successfully applied to various Physical Therapy Schools. A trend within Physical Therapy is for degree programs to be at the Master's level. There are five Physical Therapy programs in Georgia.

Recreational use of the facilities is coordinated through KSU Intramural & Recreation Services (770-499-3207) and the KSU Center for Health Promotion and Wellness (770-423-6394).
12. I am interested in athletics at KSU (e.g., teams, schedules, scholarships, etc). Whom do I need to contact?
