Deadlines:
ALL required forms, listed above, are required by the listed due date for the semester in which you are doing your Clinical.
Summer 2007 - April 18th, 2007
Fall 2007 - July 12th, 2007
Spring 2008 - November 29, 2007
Summer 2008 - April 17, 2008.
If any of your credentials (ie - PPD, CPR, Physical) are due to expire during a semester (regardless if done within the year), you are required to update it before the above listed deadlines for that semester. It is the student's responsibility to find out what Medical information is pending.
