• All instructors must have a Background Check & Drug Screening through Advantage www.advantagestudents.com.  The fee for this service is $97.00. Select DeKalb Medical to view your results.  Background Check & Drug Screening must be completed at least two weeks before the first day of clinical.   Forward e-mailed Advantage payment receipt to llongeir@kennesaw.edu for reimbursement.
• Review Training Powerpoint and complete Sunrise Clinical Assessment and submit
• Complete Clinical Orientation Check off Sheet and submit.
• Review and complete User Access and Confidentiality Agreement and submit.
• Review and complete EXHIBIT B Statement of The Joint Commission on Accreditation of HealthCare Organizations and submit, please have a witness to print and sign their name before submitting.
