- Required Immunizations:
- Proof of Tetanus
- Proof of Varicella Titer report
- Proof of Hepatitis B Titer Report
- Proof of MMR Titer report
- Negative PPD within one year OR negative chest X-Ray within twelve months OR negative Quantiferon
- Front and back of CPR card
You can submit the first page of your physical if fully completed with current immunizations

- 3 Certificate of Completion: 1.) VHA Mandatory Training for Trainees 2.) Privacy & HIPAA Training 3.) Safe Handling for Direct Care Providers
