Certified Nursing Assistant (CNA) Application Process

In order to apply for our Certified Nursing Assistant (CNA) program, you will need to complete several forms, including a notarized background check consent form, and a TB skin test. Once your forms are completed, please mail or bring all original forms and TB skin test results to:

Please download, print, and complete the following forms:

1. Application Form
2. Background Check Consent Form (must be notarized)
3. Immunization Form
4. Hepatitis B Form
5. Release, Waiver of Liability and Covenant Not to Sue Form

All paperwork, including application, background check, waiver and immunization forms MUST be turned in one week prior to class start date.

Note: Acceptance of registration is contingent upon completion of the application process. This includes a cleared background check and passed drug test. If you do not meet these requirements you will be issued a full refund.
