Supervisor Evaluation Form -- Please fill out this online evaluation of your KSU student teaching supervisor. When you fill out the form and click on the submit button, you will be taken to a confirmation page. Please print out that page and give it to your KSU supervisor as proof that you have completed his/her evaluation. The process is anonymous, and the confirmation page does not give your name, but instead refers to you as the candidate.
