ONLINE GRADUATE APPLICATION

Use this form to:

• Apply to the Graduate College for a Masters or Doctorate program

• Apply to "readmit" if you have been inactive for more than 6 consecutive semesters

GRADUATE TRANSIENT APPLICATION

Use this form if:

• You are currently enrolled in a graduate program at another institution and wish to take a graduate course at KSU that will be counted at your home institution.

UPDATE FORM

Use this form if:

• You were accepted but did not enroll and would like to be considered for a future semester

• You have submitted an application but now need to change information on that application

CHANGE OF MAJOR/CONCENTRATION

Use this form if:

• You are a currently enrolled Graduate student and would like to request to change your major or your concentration

GRADUATE CERTIFICATE IN LEADERSHIP AND ETHICS
INTENT TO ENROLL FORM

Use this form if:

• You are planning to enroll in the Leadership and Ethics Graduate Certificate program.

• Print the form, complete the form and send it to the Office of Graduate Admission

Request for Waiver of TOEFL/IELTS Test

Use this form if:

• You would like to request that the TOEFL/IELTS test be waived

• Print the form, complete the form, gather required supporting documentation and send packet to the Office of Graduate Admissions
