have been inactive for more than 6 consecutive
semesters
- Apply for the next highest level degree program (example: going from a Masters to a Specialist or Doctorate)
Use this form if:

a graduate program at another institution and
wish to take a graduate course at KSU that will
be counted at your home institution.
Use this form if:

You were accepted as a
Graduate student but did not enroll and would
like to be considered for a future semester

You have submitted a Graduate
application but now need to change information
on that application

You are a currently enrolled
Graduate student and would like to request to
change your major or your concentration
Use this form if:

You are planning to enroll in
the Leadership and Ethics Graduate Certificate
program.

form and send it to the Office of Graduate
Admission

You would like to request
that the TOEFL/IELTS test be waived

and send packet to the Office of Graduate
