How do I declare a major in the Department of Health, Physical Education, and Sport Science?
How do I find out who is my academic advisor?
All new students seeking academic advisement should contact HPS Advising Center (CC 3006 or 770-423-6544) when they officially declare their major of study. The Advising Center will assign each new major a faculty advisor.
Whom should I contact to request to be added to a closed class outside of the Department of HPS (e.g., ENGL 1101, MATH 1101, etc)?
Whom should I contact to request to be added to a closed class within the Department of HPS (e.g., HPS 1000, HPS 3200, etc)?
Contact the HPS Department's Central Office (CC 3003 or CC 3015) for the departmental form. Have your advisor sign the form and then return it to the HPS Department's Central Office (CC 3003 or CC 3015). Forms will not be accepted via FAX or electronically.

Where do I pick-up forms for Program Admission, Student Teaching, Practicum, Internship, etc.?
Does KSU offer a Physical Therapy Program?
KSU does not offer a degree program in Physical Therapy. However, students often attend KSU majoring in either Exercise and Health Science or Biology. Following completion of this degree, students have successfully applied to various Physical Therapy Schools. A trend within Physical Therapy is for degree programs to be at the Doctoral level. There are five Physical Therapy programs in Georgia.
