Request to Place Material on Reserve

Faculty must submit a separate request form for each item they wish to place on Reserve.

Faculty wishing to place an item on electronic reserve must fill out the Fair Use Checklist for each item. Any item lacking this information will not be placed on electronic reserve. For more information about the Fair Use checklist and how it applies to Electronic Reserves, consult the University System of Georgia's Introduction to the Fair Use Checklist and http://www.usg.edu/copyright/site/additional_guidelines_for_electronic_reserves/.

The completed form(s) can be sent via email or campus mail. Faculty and staff are asked to provide all source materials, including folders, within two days of submitting the information.
