NOTE:

THE COMMITTEE WILL NOT CONSIDER AN APPEAL UNTIL ALL PAGES OF THIS FORM ARE RETURNED TO THE REGISTRAR'S OFFICE. THEY MUST BE COMPLETED FULLY AND CORRECTLY.
I. ACADEMIC DISMISSAL
FIRST DISMISSAL Must remain out of school for one semester ( or Term) before being eligible to return.
SECOND DISMISSAL Must remain out of school for one calendar year or two semesters and a Summer Term after dismissal before being eligible to return.
THIRD DISMISSAL Third dismissal is FINAL. A student is only eligible to return in very extraordinary cases. These cases must be reviewed by the Academic Standing Committee. A student may appeal after the lapse of one calendar year.

According to University policy, dismissal is automatic when a student's grade point average falls below the minimum specified in the Catalog. Exceptions to this policy are granted very infrequently and only in cases where the student can demonstrate extraordinary circumstances at the time of the academic difficulty was encountered. Such circumstances as "working too many hours" or "personal problems" are really insufficient reasons for an exception to minimum standards being granted. If you believe that your situation is unique and truly involves "extraordinary circumstances", then you may petition the Academic Standing Committee.

The Academic Standing Committee will consider your appeal based upon the information you submit on the attached form (along with any additional credentials that you wish to submit). The committee will not interview students; therefore, it is necessary to be thorough. After the Committee has reviewed your appeal, the decision will be rendered in writing. If you wish to know the results immediately after the meeting, you may call the Office of the Registrar (770) 423-6200.

Financial Aid recipients should be aware that this appeal is for re-admission only. A successful appeal for readmission does NOT mean that financial aid is reinstated. Students wishing to appeal for reinstatement of financial aid must do so in writing to the Financial Aid Office.

NOTE: Students who have been dismissed for any reason must apply for readmission by completing the application for readmission form in the Office of Admission and must re-apply by the appropriate deadline.
II. GRADE CHANGE - A student who feels a grade of "WF" or "F" (for failure to withdraw properly) was unjustly assigned may appeal to the Academic Standing Committee. The Academic Standing Committee can only review circumstances dealing with grade assignments for withdrawals, all other grade appeals should be handled as outlined in the Catalog. Before the Committee is willing to consider this change, THE STUDENT MUST HAVE THE INSTRUCTOR COMPLETE THE SUPPLEMENTAL CONSENT FORM ON THE BACK OF THIS APPLICATION!
III. DEVELOPMENTAL STUDIES EXCLUSION - A student who does not complete a Developmental Studies area in two attempts shall be placed on exclusion.
IV. NEW STUDENTS (first time applicants) requesting appeal consideration MUST obtain all applicable admissions documents to be heard.
IT IS THE STUDENT'S RESPONSIBILITY TO SEE THAT THIS PACKET IS COMPLETE WHEN RETURNED TO THE OFFICE OF THE REGISTRAR
