About the Center
The
Department of Student Life is located in room 274 of the Carmichael Student
Center. The Department of Student Life is responsible for all registered student
organizations (over 150!), student media, intramural sports and recreation
services, and student leadership programs. The Department of Student Life is
also responsible for the operation of the Information Booth; management
of the student activity budgets; maintenance of organization mail folders;
and handling reservation requests for student organizations.
The Department of Student Life
staff includes the director, associate directors, assistant directors,
business manager, administrative secretary and administrative assistant.

The
Activity Request Form
All fields must be filled
out before you can submit the request. Please fill out the request for
the current semester. Future semester requests should be on a separate
request. You will receive confirmation and room assignment via e-mail.
A hard copy confirmation will be sent to your advisor with room assignment.

Other Student Life Forms & Applications
The Information
Booth
Located on the first
floor of the Carmichael Student Center, the Information Booth is
operated under the direction of the Department of Student Life and is staffed
by student assistants.
General information
is available on a variety of subjects. You can pick up printed material
such as schedule of classes, student handbooks and monthly calendars of activities. Information Booth workers can answer your questions or tell you whom to call or see to find answers. Registered student
organization mailboxes are located in the Information Booth. The booth
is open every weekday and some weekends. Please call 423-6280 for hours.
Lost and found services are available at the Information Booth. Items found on campus should
be turned in there. You may recover found items at the KSU Police office
as well as at the Information Booth.
Markers and paper
for making banners are available free of charge to student organizations
and departments in the Department of Student Life, room 274 of the Student
Center. For more information concerning the booth call 770-423-6280

KSU
Identification Cards
KSU ID cards are now
handled by the ID Card
Universal Card Services Center in the student center Room 219 (above
the dining area). Call 770-499-343.

Market Day
Market Day is
an opportunity for vendors, businesses, and members of the
local community to offer information, services, and products to KSU
students in a open market atmosphere. Click
