Areas Addressed by Case Management Team

Monitoring	Periodic meetings to reassess new needs and/or review  ongoing progress Follow-up to ensure the referrals and/or the information provided met needs
Referral/Bridging Services	Links to ongoing treatment and/or community resources During termination phase with CPS therapist, connect students to needed community resources
Behavioral Health Follow-up	Provide follow-up after inpatient and/or residential treatment Advocate on student’s behalf to University, significant others and/or community
Crisis Case Management	Assess students’ capacity to cope during and/or following a crisis Prioritization of needs and development of action plan Coordination of care with both formal and informal supports
