EHS Requests & Approvals

All KSU employees and staff should submit requests through the University’s online system, EtQ Reliance, using the EHS Approvals module. Use this module to submit requests for campus events, PPE (safety shoes), medical services, hot work permits, equipment purchases, and decontamination. Submit requests as soon as possible after identifying the need.

Use the following forms for:

  • Event planning that requires University approval or campus resources.
  • Requests for personal protective equipment (e.g., safety shoes).
  • Occupational medical evaluations, testing, or follow-up after exposures or incidents.
  • Hot work that produces sparks, flames, or heat (permits required before work begins).
  • Equipment purchases for specialized, high-value, or safety-sensitive items.
  • Decontamination of equipment, spaces, or materials exposed to hazardous substances.
     
    Serious incidents or immediate hazards should be reported immediately by calling the campus emergency number 470-578-6666.
